  • Household Income Form (HIF)

  • Permission Form for Medication (if applicable for each medicine)

  • Immunization Records (Please make sure record is current)

  • Physical (Initial and before entering 6th Grade)

  • Dental and Vision Exam

  • Birth Certificate or I-94

  • Custody Papers (if applicable)

  • Home Language Survey
