- Student Emergency Card
- Infinite Campus Parent Portal
- Call IC Clerk for Username and Password
- Household Income Form (HIF)
- Migrant Form
- Transportation Form
- Permission Form for Medication (if applicable for each medicine)
Initial Documentation:
- Immunization Records (Please make sure record is current)
- Physical (Initial and before entering 6th Grade)
- Dental and Vision Exam
- Birth Certificate or I-94
- Custody Papers (if applicable)
- Home Language Survey
